Participant Information
 
 
 
In consideration of the above-named participant being allowed to participate in events or activities of United Indoor Soccer, Inc., the parent/guardian with legal responsibility, acknowledges, appreciates and agrees that: 
  1. I agree to indemnify United Indoor Soccer, Inc. , its principals, employees, agents and sponsors, and all other participants of activities within the United Indoor Soccer, Inc. facilities from any and all liability that may arise out of, or related to any physical injury, loss or harm that may occur to the above-named participant while participating in any activity within the United Indoor Soccer, Inc. facilities.
  2. I am aware that there are inherent and other risks involved in the sport of indoor soccer, and that physical injuries are a common and ordinary occurrence of this sport, and I knowingly and freely assume such risk on behalf of the above-named participant.
  3. I understand that the above-named participant is expected to comply with all posted and customary terms, conditions and rules for participation for the sport of indoor soccer, and is to conduct himself/herself in a sportsmanlike manner at all times within the United Indoor Soccer, Inc. facilities, both on and off the playing field.
 
I have read this Participation Agreement and Release of Liability, and I fully understand its terms and acknowledge that I will be giving up substantial rights by signing it and that I have signed it freely and voluntarily without any inducement or coercion.
I agree that the requirements of this Participation Agreement and Release of Liability shall apply both this year and any future years during which the above-named minor participates in activities within the United Indoor Soccer, Inc. facilities.
